I suggest that a certain indiscriminate segment of society, who normally would take little notice of science or medicine, will look at him and identify with him – “he’s one of us!” Thus, he will attract and influence an otherwise apathetic section of the propagandised public.
You wonder why people like Hotez are becoming more and more unhinged on this issue. It can't be purely that it threatens his personal income stream. His angst seems to run much deeper; that a collapse in vaccine confidence may result in pressure to examine this industry, exposing its criminality, and his own willing participation in it, potentially leading to his incarceration. (Sadly, I don't think accountability will ever happen. But I hope the possibility he might one day face jail time keeps him stressed and sleepless).

In your second sentence, "perhaps billions" - I feel credibility drop off a cliff. I don't think we're anywhere near 1/8 to 1/4 of the world's population having died from the big V. Is it still possible? Maybe, but at this point doubtful. And then to go on with personal attacks on his physical size etc. They may at times be a fair consideration given that he's a doctor but do nothing to add to the pertinent story of his outrageous statements. I can't stand the lack of principles with the left's "journalism," I equally don't like similar stuff from "our" side, or any "side."
The physical condition of a physician matters greatly and is always an important consideration. If you don’t think so you’re forgetting what their proposed role is. You’re also then not taking into consideration that his poor state of health constitutes yet another motive— in addition to grift, greed and hunger for power — to push biomedical products on the world’s population.
